About Us: What to Expect At Your Appointment

Before the Visit…

Please telephone Ballert as soon as possible after receiving your prescription or referral to schedule your appointment Our experienced schedulers will help you to:

  • Determine the care you require and assign you a practitioner most suited to your needs;
  • Schedule an appointment at a facility most convenient for you;
  • Take some information over the phone to help us expedite the check-in process on the day of your appointment. Information you may be asked to provide includes address, phone numbers, date of birth, social security number and insurance information.

Ballert will verify your insurance benefits and coverage prior to your appointment. This allows us to notify you of any problems in advance, so that we can work with you, if necessary, to resolve them. We encourage our patients to take the time to read and understand their policies because ultimately you are responsible for your medical bills.

Your First Appointment…

The day before your scheduled appointment, we will call you to confirm the time and location of your appointment. If for some reason you are unable to keep your appointment and need to reschedule, we ask that you contact us as soon as possible.

In order to ensure prompt treatment, please remember to bring the following information and documents with you at the time of your visit.

  • A complete and signed insurance form and card
  • A prescription and/or HMO referral
  • The name and telephone number of the insurance carrier
  • The name and telephone number of the insurance representative who is familiar with your case.
  • You may be asked to complete an “assignment of benefits” form authorizing the insurance company to pay us directly.

Payment and Insurance

Ballert Orthopedic is a provider of services for many insurance carriers. Please contact us if you are in doubt about whether we accept your insurance.

We will make every effort to verify your insurance coverage prior to your first visit, and we will discuss with you any portion of the fee that is not covered because of deductibles, co-pays or limitations in your policy’s coverage. You will be asked to sign a form called “Assignment of Benefits” which authorizes your insurance company to remit payment directly to Ballert. When your insurance settles the claim, they will include and “Explanation of Benefits” (EOB), showing what portion of the bill remains your responsibility to pay. Upon settlement of the primary insurance claim, Ballert will either forward a claim to your secondary insurance, if applicable, or send you an invoice for the balance due. If you have any questions regarding your invoice, we encourage you to contact our accounts receivable department as soon as possible.

If your portion of the fee will lead to financial hardship for you, please ask to discuss your situation with one of our reimbursement professionals. We will be happy to work with you on a mutually satisfactory arrangement. If you qualify for a payment plan, Ballert will send you an invoice every month.

Ballert may refuse direct insurance billing for repairs and modifications for off-warranty devices and for some services. You will be advised as to this restriction and given an estimate of the charges prior to the services being rendered.

Medicare:

Ballert will assist you in submitting claims and receiving the maximum benefits. Ballert accepts Medicare assignments routinely. If a Medicare assignment is accepted, you are responsible for 20 percent of the approved amount, as well as any deductible that is not met. Ballert routinely submits claims to Medicare on behalf of our patients.

Worker's Compensation...

Ballert Orthopedic also accepts Worker’s Compensation and Medicaid.

What You (the Patient) Can Expect to Pay...

If an item is not covered by your insurance you will be asked to pay half of the fees at the time of evaluation and the balance at the time of delivery. If the item is covered by your insurance, you may be asked to pay at the time of service the portion that is your coinsurance.
